Patient Portal
NextMD and MedFusion are secure, password protected patient portal services. They allow you to view your records and to contact our office about non-urgent questions through a secure web portal at your convenience.
Nextmd Existing Account
To view your records, from the HOME page hover over the MY CHARTS tab at the top, click on Request Health Record and then submit. You will receive your health record in the patient portal within 24 hours. An email will alert you when your record is received or if we have sent you a message through the portal.
New Account Signup
You must have a valid email address on file with us to enroll in the portal. If you would like to enroll, simply call our office, verify your email address and we will give you a temporary user name and password. When you receive an email indicating your enrollment is complete, you can follow the link in the email, sign in with the temporary credentials, reset your user name and password.
MedFusion Existing Account
